Application for Entry into FHS Database

This form is for entering your already registered horse (with another UELN registry) into the FHS database (so their offspring can be registered with FHS). Please mail, email or fax a copy of the horse’s registration paper to the FHS office.

$25 fee.

Dwarfism & Hydrocephalus testing may be required for Stallions and Mares if not already on file. (Additional fee of $60)
